Our  new system is a convenient way to verify our patients online.
This system will allow patients to be verified 24 hours a day.

Our ID system is keyed to each patients recomendation. Each patient will have a unique verification USERID located on the recommendation.

Current patients who wish to be enrolled in our online verification should email us cc4wellness@yahoo.com

Note: Our database does not contain actual names or contact information of any patient for confidentiality reasons.
